National Jewish Health Chronic Care Management^TM is an easy-to-use tool that helps you take a proactive approach to your health so small problems don’t become big ones.

Hi, I'm Dave Buther, Chief Medical Information Officer and one of the lung doctors here at National Jewish Health.

I'm excited to introduce a new tool for our Colorado patients called Chronic Care Management.

This tool is really easy to use, connects you closer to your care team, and helps you take a proactive approach to your health so that small problems don't become big ones.

With Chronic Care Management, your care team remotely monitors changes in your health, and they'll contact you if they see anything out of the ordinary.

It's a really helpful way for us to check on you between clinic visits.

We're introducing this new tool in October in various clinics throughout the hospital, including Cardiology, Severe Asthma, COPD, Interstitial Lung Disease, Pulmonary Hypertension,

and Rheumatology.

And, we'll be adding additional clinics soon thereafter.

Ask your care team how to get set up with Chronic Care Management.

It works with your smartphone, tablet, or computer.

Helps you stay connected to your care team in between office visits

Some patients also will check vital signs daily with a smart device like a pulse oximeter, scale or blood pressure monitor that automatically uploads your numbers.

Insurance Coverage

Participation in the National Jewish Health Chronic Care Management program is covered by Medicare and most insurance plans. Standard deductibles, co-pays and co-insurance may apply. Please check with your insurance provider to verify your coverage.

Getting Started

Talk with your care team to determine if this program is right for you. Please note that this program is only available to patients who reside in Colorado.

Unenroll at Anytime

If you decide this program is not right for you, you can ask a care team member to unenroll you from Chronic Care Management. Please note that this is a month-by-month program. If you unenroll before the end of a month, there may be outstanding charges that will be billed after unenrollment.
