If you would like to make a request to amend your medical records, please complete the Request for Amendment of Health Information Form and return it to the Health Information Management department at National Jewish Health.
Download, complete, and print the Request for Amendment of Health Information Form, making sure to include your handwritten signature.

Medical Record Destruction Notices
On January 29, 2025, National Jewish Medical and Research Center, 1400 Jackson St, Denver, Colorado, 80206, will destroy all paper pediatric medical records of persons, born 01/01/1996 and 12/31/1996, who received treatment at National Jewish Medical and Research Center prior to 01/01/1996-12/31/2009. Please direct inquiries concerning records to the Health Information Management Department (303)398-1580 between 8:00am and 4:30pm, Monday-Friday. Inquiries for the entire paper record will not be accepted after January 27th, 2025.
